We appreciate your selection of our office to serve your child’s dental needs. We will do all we can to provide him/her with the best care. Please read this letter carefully. In it we hope to give you the necessary information about our practice and policies which will assist us in serving your child’s dental needs, avoiding unnecessary frustrations and misunderstandings. We take great pride in our training, abilities and dedication and hope that you will soon share our confidence.

Office Hours

The office is open to serve you, answer your questions, or schedule an appointment during the following hours:

Monday, Wednesday and Thursday: 8:00am to 5:00pm
Saturday: 8:00am to 12:00pm

We are closed on Tuesday, when the doctor teaches at the University of Illinois-Chicago, Friday, and major holidays. Dr. Seidman is always available for emergencies. Should you need the doctor when the office is closed you can reach him by phoning the office and listening to the emergency instructions or calling the emergency phone number directly at 847 525-7890. Please don’t leave an emergency message on the office’s voice mail.

The office attempts to schedule appointments at your convenience. Preschool children should be seen in the morning because they are fresher and we have the time to work more slowly with your child. This allows us to keep them more comfortable and affords us the extra time that their age demands. School age children who require more involved treatment are also better served by being seen in the morning for the similar reasons. Dental appointments are an excused absence. Missing school can be kept to a minimum when regular dental care is continued.

Cancelling Appointments

We ask that if you cannot make your appointment with us, to please call at least 24 hours in advance. We may be able to use the time that was reserved for you in a way that could benefit another patient. If an appointment is cancelled within less than 24 hours or an appointment is broken a $25 fee will be applied to your account.

Financial Arrangements

In an effort to hold the line on dental fees while maintaining a superior level of professional care we have established the following payment options:

  1. Payment in full or payment of the portion the insurance carrier will not cover including co-pays, on the day treatment is rendered.
  2. Payment of balance in full upon receipt of statement.
  3. Payments can be made using cash, check, MasterCard, Visa or Discover Card.

Payments can be made using cash, check, MasterCard, Visa or Discover Card. The parent who authorizes dental care is responsible for payment. Payment plans are available upon request. As an added consideration, accounts outstanding for over 60 days will accrue a billing fee of 1½% per month. If an account is sent out for collection any costs accrued will be the responsibility of the past due account holder. Your child’s dental health and continuing follow-up is our primary concern.

Dental Insurance

We participate in the insurance plans offered by most major companies. Our services almost always fall into the “usual and customary fees” for pediatric dentists in our area. Please contact our office for the names of those companies with whom we are “in network”. Regardless of insurance carrier we are prepared to file claims on your behalf. We ask that you keep us up to date with all of your insurance information and let us know immediately if there are any changes in either carriers or plans. Please be familiar with the terms of your insurance policy. We will work with you to determine your insurance reimbursement for any treatment that might be necessary. A pre-treatment estimate can be submitted if any extensive treatment is required. Please be prepared to pay any co-pays or the amount that we anticipate won’t be covered by your carrier the day treatment is rendered.

Payment Plans

Sometimes, in your child’s best interest, it will necessary to begin treatment before you are prepared to make payments at the time services are rendered. We participate in a six month, no interest payment program with CareCredit. Extended plans are available as well. Please ask us for information about this option. We will explain how you can register for and use this service.

Continuing Care (Recall/Cleaning)

Children are dynamic and constantly changing. Their oral status and dental condition is changing as well. Appropriate recall, every six months, has shown to be very helpful in maintaining a high state of dental health. Upon completion of their treatment we will place your child in our continuing care (recall) program. These appointments are designed to monitor their dental condition, note any dental changes and treat those needs.
